SIP data is transmitted through LTE network using specific QCI bearer i.e QCI "1".
Protocol stack at UE side receives RTP based IP packets at QCI "1" radio bearer and send to IP layer of UE and it is delivered to correct application based on port.

In case of UL UE IP layer packs an SIP packet and sends to lower layer but how UE stack knows it has to forward to QCI 1 bearer, because for UE stack it is just an IP packet.

Different bearers created for different qci can be differentiated using TFT. Every bearer have DL and UL TFT. UL TFT is enforced by ue. TFT can have port range or destination IP etc. So if ue is having sip data to send in UL and it is expected to go through qci 5 bearer then ue will match TFT . If TFT is matching with qci 5 bearer then ue will send with qci 5 bearer otherwise he will send with default bearer.
