How PCI selection is done for a lte cell ?

Is there any formula being applied while choosing PCI for a lte cell or any random value in range of 1-504 can be used ?

We have a formula to calculate PCI=3*(PCI group)+cell identity in group

When we switch on UE it will scan and tune to its radio frequency depending on which band it is supporting Now suppose ue is tuned to a particular frequency channel, it will try to find PSS and SSS.

From PSS UE will get cell id in group ranges from 0 to 2
Fron SSS UE will get physical cell id group ranges from 0 to 167

I went through 36.331 spec, there are various terms such as Srxlev, Q-hyst and so on are used.
Can someone please give a brief overview of all the terms used during the cell selection and cell re-selection procedure ?

If my understaing is correct then in case of carrier aggregation(CA), an UE can connect to multiple cells. One of the cell is known as primary cell while other cells are known as secondary cells. Primary cell physical layer failure is detected based on the receiving N310 consecutive "out of sync". Is the same case applicable for secondary cells as well ?
